Iowa Care Management, LLC (ICM) provides mobile urgent and emergent healthcare services under medical direction and approved clinical protocols. Services are designed to support local EMS systems, improve access to care, and reduce unnecessary emergency department utilization.

ICM operates in coordination with local stakeholders and in alignment with Iowa EMS regulations and scope of practice requirements. Our services are structured to “support” not “replace” traditional EMS response and are designed to integrate with existing 911 systems, dispatch protocols, and regional medical control.

Iowa Care Management is seeking a Physician Medical Director to oversee a growing Mobile Integrated Health and Community Paramedicine program. This is a low-burden, high-impact leadership role supporting in-home care, MIH services, chronic disease management, STD testing and treatment, mental health response, and EMS system partnerships.

What You’ll Do

  • Provide medical oversight and protocol approval

  • Support paramedics operating in community and home settings

  • Participate in QA and limited case review

  • Advise on MIH expansion and 911/EMS partnerships

Who You Are

  • MD or DO licensed in Iowa (or eligible)

  • Comfortable with EMS and non-traditional care models

Medical Director


The Healthcare Technician functions as a licensed healthcare professional delivering patient assessment, treatment, monitoring, and care coordination within their authorized scope of practice and under medical oversight.

This position supports urgent and emergent patient care responses, scheduled clinical visits, and system support operations when deployed in coordination with 911 services or partner agencies.

Minimum Qualifications (Required)

Applicants must hold one of the following active, unrestricted licenses:

  • Paramedic

  • EMT (EMT-B or AEMT)

  • Registered Nurse (RN or LPN)

  • Licensed Respiratory Therapist (RT)

  • License must be valid in the State of practice

  • Current BLS certification (ACLS preferred where applicable)

  • Valid driver’s license with acceptable driving record

  • Ability to meet physical demands of field-based clinical work

Scope of Practice & Responsibilities

All duties are performed within license scope, state regulations, and medical director–approved protocols, including:

  • Conduct patient assessments and clinical evaluations

  • Provide urgent and emergent medical care as authorized

  • Administer medications and treatments per standing orders

  • Perform approved point-of-care testing and vital sign monitoring

  • Coordinate patient referrals, follow-up care, and education

  • Support EMS system operations when requested and authorized

  • Maintain accurate, timely electronic medical records

  • Adhere to infection control, safety, and quality assurance standards

  • Participate in required training, protocol review, and QA/QI activities

Preferred Experience

  • EMS or emergency care experience

  • Community Paramedicine or Mobile Integrated Health programs

  • Public health or chronic care management

  • Experience working under standing orders and remote medical oversight
